Patients should obtain a Registration form from
Reception and sign and complete appropriately. This
should be returned to reception as soon as possible so
that we can obtain your medical records from your
previous practice.
We operate a system in the Practice whereby you can see
any of the General Practitioners and not solely the
Doctor whom you are registered with.
The best way to receive excellent medical care from our
doctors is to choose one, and to stay with them, as the
long term relationship and knowledge of your medical
history is invaluable. Switching doctors frequently can
be rather confusing and frustrating, both for you and
the doctor, as you will need to repeat your problem and
start from scratch every time, though your medical
records can help the doctor know what has been happening
to you. We quite understand that there will be times
when you need to see any doctor quickly for an urgent
problem, but the value of this has to be balanced
against continuity of care.
Newly Registered Patients
All newly registered patients will be asked to complete
a questionnaire and have a consultation with the
Practice Nurse for a small new patient medical to be
completed. This involves a blood pressure, height,
weight and urine check.
Medical treatment is available from the date of
registration.
